To register for Attorney of Record access, forms must be either notarized by a Notary or executed by a Deputy Clerk. Visit any Clerk’s office location to have a Deputy Clerk assist you. The Deputy Clerk will ask for your picture ID (and bar card, if you are an attorney) and for you to sign the form in front of them.
