Office Holiday Closure Notice: The Clerk's office will be closed on Monday, October 14 in observance of Columbus Day.

 

After hours payments on traffic or criminal cases that are not in collections will be available from 10 a.m. to 2 p.m. on Saturdays and Sundays and  from 9 a.m. to 6 p.m. on Monday, October 14, via the nCourt payment system at 561-328-2463. Please have your citation, case or driver’s license number available when placing your call.

 

The E-Filing portal will be unavailable due to maintenance from 4 – 7 p.m. on Sunday, October 13.

Apply for a passport at the following Clerk & Comptroller branch locations. Note: Passport applications are not accepted at the Main Courthouse nor the Governmental Center.

No appointment is necessary. Open Monday through Friday, 8 a.m. - 4 p.m. except designated holidays.

Check wait times at each branch for civil court services, such as passports and marriage licenses.

Passport photos are also available.

Passports are issued by the U.S. Department of State.

New Applicants

You must apply in person and complete the Passport Application Form DS-11 if any of the following are true:

  • You have never had a U.S. passport
  • You are under age 16
  • Your most recent passport was issued more than 15 years ago
  • Your most recent passport was lost or stolen
  • You are renewing a five-year passport

Please bring with you the following required documentation: Proof of Citizenship, Proof of Identity, One Passport Photograph, one Check or Money Order for the appropriate amount made payable to the U.S. Department of State and one other method of payment for the passport execution fee made payable to Clerk & Comptroller, Palm Beach County.

Proof of Citizenship

Provide one of the following:

  • Previous U.S. Passport
  • Certified copy of your birth certificate (that includes names of both parents)
  • Certificate of Naturalization

Proof of Identity

Provide one of the following:

  • Previous U.S. Passport
  • Valid Driver's License (A temporary license does not meet this requirement.)
  • State Identification Card (Photographic likeness should be recent.)
  • Certificate of Naturalization

If you cannot prove your identity as outlined above, you may use an identifying witness to supplement secondary documents.

  • An identifying witness must:
    • Be at least 18 years old
    • Be a U.S. Citizen or permanent resident alien
    • Be able to present acceptable primary identification
    • Be able to attest that they have known the applicant for at least two years.
  • The witness must appear with the applicant and will be required to complete and sign Form DS-71 prior to execution of the passport application.

Passport Photograph

One passport photograph is required.

  • Recent photo (Likeness of applicant)
  • Front view/full face
  • No eye glasses
  • 2-inch by 2-inch in size
  • Photo must be in color

Photo may be purchased at the Clerk's office for $12 plus tax.

Minor Applicants (Under Age 16)

Children under age 16 (For new and renewal passports)

You must complete the New Applicant DS-11 form.

Minors must be present when applying.

Parental Consent

Both parents must appear together to sign the application DS-11 form. If this is not possible, one parent must be present to sign the application, and the absent parent MUST submit a completed and notarized "Statement of Consent" Form DS-3053 AND a photocopy (front and back) of their ID. The notarized consent form cannot be more than three months old.

If one parent or guardian cannot locate the other parent or guardian, then you must submit a Statement of Exigent/Special Family Circumstances form DS-5525 in addition to the child’s application. This form may also be used if the applicant’s family situation makes it exceptionally difficult or impossible to provide notarized, written consent.

Passport Renewal (By Mail)

Renew Your Passport by Mail (PDF)

All passport renewals require the completion of Form DS-82 and are only renewed by mail. You must renew your passport by mail if:

  • You hold a passport and it is not more than 5 years past the date of expiration and not more than 15 years since being issued.
  • You were at least 16 years of age when it was issued.

Instructions:

  • Complete, sign and mail Form DS-82.
  • Include your most recent passport with a new passport photo.
  • If the name you are using differs from the name on your most recent passport, you must submit one of the following:
    • Certified copy of the marriage certificate
    • Certified copy of court order showing the change
  • If your passport is mutilated or damaged, you must do both of the following:
    • Use the DS-11 application form
    • Apply in person at one of the acceptance agencies with a certified copy of your birth certificate (must have the names of both parents on it).

Passport Reapplication

If your most recent passport was issued less than 1 year ago, use DS-5504 form for the following:

  • Change of name
  • Correction of descriptive date
  • Extend the validity of a Limited Passport

Instructions:

Note: There is no fee if your passport was issued less than 1 year ago, unless you request expedited processing.

If your most recent passport was issued more than 1 year ago AND you have the passport, use Form DS-82.

  • Complete the DS-82 form.

If your most recent passport was issued more than 1 year ago AND you do not have the passport, use DS-11 form.

How Long Passports Are Valid

  • Adults (16 years of age and older) - Ten (10) years
  • Children (15 years and younger) - Five (5) years

Check the Status of Your Passport Application

Call 1-877-487-2778 from 6 a.m. - Midnight EST, Monday through Friday (excluding federal holidays), or check online at the Department of State website.

Customers Traveling Within Four Weeks

If you are traveling within 14 calendar days, you must make an appointment at the nearest Passport Agency by calling the National Passport Information Center at 1-877-487-2778.

If you are traveling within 2-4 weeks, you must request and pay for expedited service plus two-way overnight delivery service. For current expedited service fees, see Passport Fees & Costs.
