Yes. The original “Notice of Appeal” is required, along with your payment.
You must provide a copy if you want a “stamped” copy from the Clerk & Comptroller for your records.
The Clerk & Comptroller will make a certified copy of the original and will send your “Notice of Appeal” along with your payment to the 4th District Court of Appeal.
These cases are appealed to the 4th District Court of Appeals (DCA).
These cases are appealed to the Circuit court.
